Please help! It's about my er Spanial!?

It is hard to know what could be causing it. You probably should go back to the vet or try a new one. But it does kind of sound like an infection or possibly a secondary infection caused by an allergy. Odd smells are usually a sign of an infection and since neosporin helped then it may be bacterial. Something that would definitely help that is reducing carbohydrates in his diet as they help spread bacteria and yeast, the most common caused of an infection.
